    • Patients can book appointments by calling the office once referred to the clinic by a hospitalist, community agency, or other health care providers.

    • Not currently accepting new primary care patients for ongoing care. If you are looking for a family doctor or nurse practitioner please register online here.

    • This clinic is operated by nurse practitioners and physicians providing a wide range of health care services for the post-discharged care of patients/clients without a family physician in the community and government assisted refugees.

       

      Referrals:

      • Patients/clients recently discharged from the Abbotsford Regional Hospital who do not have a family physician in the community can be referred.
      • A referral can come from a hospitalist, community agency, or other health care providers.
      • Nurse practitioners and physicians provide temporary post-discharge follow-up care through physician referral.
      • Government assisted refugees can be referred through community agencies.
