- Not currently accepting new primary care patients for ongoing care. If you are looking for a family doctor or nurse practitioner please register online here.
- This clinic provides primary care services to a specific population of patients, which includes those with complex psycho-social and clinical needs. These patients are otherwise vulnerable and require a higher intensity of services to achieve functional stability.
