Looking for a Family Doctor or Nurse Practitioner?

The Health Connect Registry is a new region-wide patient registry in Surrey and North Delta for people who are looking for a physician or nurse practitioner. It replaces the Surrey-North Delta Division of Family Practice patient portal waitlist. You can register in our community if you, your family, or a person in your care lives in one of our six Primary Care Network (PCN) neighbourhoods: North Delta-West Newton, North Surrey-Whalley, Guildford, Cloverdale-Panorama, Fleetwood or East Newton.

The Health Connect Registry is the most efficient way to find a family doctor or nurse practitioner in this region. Once registered, you will be added to the list in chronological order and contacted directly when a primary care provider is available. We ask you to please be patient, as it may take some time to be connected with a doctor or nurse practitioner.

Please note: If you previously registered with the Surrey-North Delta (SND) Division of Family Practice waitlist, and agreed to have your registration transferred to the Health Connect Registry:

• you will not lose your place in line
• you will be contacted to confirm or update your registration information
